Checklist for Passport Applicants
Renewal by Mail
Prior to sending your application form, please ensure that the following documents are completed correctly to avoid delays in your application processing. Below is a summary of the requirements:
- Please enclose your current full-validity passport. We will cancel the passport when the new passport is issued and return both the cancelled passport and new one to you.
- A completed passport application form is required for each person applying for a passport. Please complete the Renewal Application Form (PC7) for adult who wish to replace passport that is current or expired less than 12 months from the day of which the passport application is signed, with at least 2 years validity on it and have not gone through a name change. Please complete the Overseas Passport Application Form (PC8) for children/1st adult passport application or people whose passports have been lost or stolen.
- Two (2) recent colour passport photographs must be submitted, one of which has been endorsed on the back by the guarantor (if you are using overseas application form). The photographs must be less than 6 months old, show the applicant looking straight at the camera, be of the head and top of shoulders only, have a light coloured background (white/light blue) and be 35-40mm x 45-50mm in size. Once it is printed, the facial measurements must be as follows: height from the crown of head to bottom of chin has to be between 32 – 36 mm. The photos should show natural expression (not laughing or frowning) with your mouth closed. If you wear glasses, it is preferred that the glasses are removed prior to the photograph being taken.
- The passport application form should be completed in black ink only.
- You must include 1 additional original photo ID (KITAS or License) if you are using the renewal application form (PC7).
- For children applications (Overseas Passport Application Form or PC8), the original of birth certificate, with full English translation, and one of the parent's passports must be submitted.
- Both parents or persons with the caring responsibility for the child, should complete the consent page of the application form. This section should be signed and witnessed by someone that is not related by marriage or birth to the applicant.
- Guarantor page (question 11 of the application form) should be completed by an individual who has known the applicant for at least 12 months or since birth if the application is for newly born child passport. The guarantor must either hold employment in one of the occupation group listed or be an Australian adult holding an Australian passport with at least two years validity. The guarantor must not be related by birth or marriage or in a de facto relationship to the applicant.
- The declaration section (on question 17) of the overseas application form should be signed by one of the parents, and if the child is 10 years or over, it is a compulsory that the child needs to sign on the signature label on question 18.
- Ensure that all signatures required on the form are signed within the provided white box provided. Failure to do so could result in delays of your application processing.
- Ensure that every box on the application form is completed with relevant information.

Priority processing fee (Emergency passport if it is overseas) A$78.00Please note that passport fees are payable in Rupiah Cash only and that the Fees are subject to change at the 1st of every month in accordance to the exchange rate at the time. So, please contact the embassy just prior to forwarding payment for the most current and correct information of the fees in Rupiah.
- Payment by bank transfer may be made to the Australian Embassy Bank Account: 082222-10001, ANZ-Panin Bank, Jl Jen Sudirman Kav 1, Jakarta Pusat. Your passport will be processed after the Embassy receives confirmation from the ANZ-Panin Bank that the funds have been received. It would also be helpful if you would fax to us (021-25505499) a copy of your funds transfer document, including your full name and the purpose of the transfer on the bank slip to quickly identify your payment.

- Please remember to write your Indonesian mailing address on the application form so we can return the passports to you. The embassy will pay for the return of your passport by local registered mail. If you would like your passport returned by courier please advise the embassy and add the courier costs to your bank transfer.
The usual processing time is 10 working days from the date of lodgement. This is subject to all application requirements being met and payment received.
